Path-Now Launches The Wayfinder Experience

On Thursday, November 9th, the San Diego Regional Center (SDRC) buzzed with energy as Path-Now proudly sponsored the Self-Determination In-Action Conference at Park & Market in San Diego, California. The highlight of the event was the innovative Wayfinder Experience, providing attendees with a unique opportunity to test Path-Now and connect with a diverse array of service providers. This interactive session aimed to empower individuals in the self-determination program, encouraging them to make informed choices and carve their own paths to a brighter future.

At the heart of the conference was the Wayfinder Experience, an immersive journey that allowed participants to explore the myriad possibilities available through Path-Now. Attendees engaged with various service providers, gaining firsthand knowledge about the services offered by organizations such as Positive Development, Options For All, Plant It Again, SELF-Start, NeuroNav, Adjoin, Adjoin Inflight, TERI Inc, Living Independently is For Everyone (L.I.F.E.), California Department of Rehabilitation, San Diego Park & Recreation, YMCA of San Diego County, Outside the Lens Media Makers, MRM Education, Toward Maximum Independence, and A.R.C.C. The Wayfinder Experience wasn’t just about connecting with service providers; it was a celebration of choice and empowerment. Participants were encouraged to reflect on their individual needs and preferences, making decisions about the support services that resonated with them the most. This hands-on approach to self-determination was a powerful reminder that individuals in the program have the agency to shape their own destinies.

Following engaging conversations with service providers, participants were not only equipped with valuable information but also received a Wayfinder Explorer Participant Patch as a symbol of their journey. As a thoughtful and inspiring conclusion to the experience, attendees were invited to leave trail end notes—personal messages of inspiration and encouragement for others on the same path.

Nonprofit Adjoin Launches Path-Now, an Innovative Application and Service Provider Network for Individuals with Disabilities

Path-Now connects individuals with Intellectual and Developmental Disabilities (IDD) with thousands of service providers throughout California

SAN DIEGO, May 24, 2023–(BUSINESS WIRE)–Adjoin, a nonprofit, charitable organization that creates limitless pathways for individuals with disabilities and veteran families throughout California, today announced the availability of Path-Now, a new, accessible mobile application that enables individuals with Intellectual and Developmental Disabilities (IDD) to safely and easily connect with community service providers electronically.

Path-Now quickly matches users to service providers that meet their unique needs and preferences. The Path-Now service provider network includes vocational programs, independent living services, transportation options, camps and recreational activities, art programs, and more. The program is currently available in California with 8,000 service providers and the team has plans to expand rapidly.

Path-Now allows individuals with IDD or their care provider to create a free profile in the application and specify the types of service(s) needed and their provider preferences. The application then instantly matches them with organizations that align with their preferences. Users can then connect with their chosen organizations directly within Path-Now to move forward in accessing needed services.

Organizations that provide services for individuals with IDD in California can claim their profile in Path-Now or request to have a profile created. Once requests are received, reviewed, and approved by Path-Now, organizations choose a subscription plan and can review and update information in relevant fields, including uploading photos and videos and gaining access to the Path-Now chat feature.

Path-Now, Where Every Individual Can Connect to the Support They Need

Path-Now is a new creative, accessible, and unique mobile application that enables individuals with Intellectual and Developmental Disabilities (I/DD) to safely and easily connect with community service providers electronically in California. Path-Now was the vision of the CEO of Adjoin, a 501(c)3 organization, who saw a critical need for there to be an interactive statewide system directory of all community service providers for individuals with I/DD to access needed services.

Path-Now provides individuals with Intellectual and Developmental Disabilities (I/DD), a family member, or a care provider, to create a free profile where the individual can answer questions related to types of service needs, preferences regarding the service or services searching for as well as preferredorganizational characteristics. Then, with the click of a button, individuals can instantly see organization profiles listed in order of percentage of match to individual’s profile responses and read about each potential service provider. Individuals can then choose to connect with organizations by clicking theconnect button for that organization. Based on if the organization has a subscription or not, the individual can either chat directly with the organization or will receive a notification to contact the organization using contact information in the organization’s profile.
